「2019～2021年（第1回～第3回） 満足度・生活の質に関する調査」の個票データについては、以下の利用条件に同意いただいた場合に提供いたします。2019年～2021年調査の約20,000サンプルの個票データを送付しております。
データ提供を希望される方は、利用条件を確認の上、下記フォームに必要事項を記入し、申請を行ってください。申請内容を確認したのち、記載いただいたメールアドレス宛てに個票データを送付いたします。
【利用条件】
１．提供された個票データについて、個々の調査対象を識別する行為は行わないこと。
２．提供された個票データについて、合理的な範囲で安全管理措置等を講ずること。
３．提供された個票データは、本利用申請をした者・所属だけが利用し、第三者に再提供しないこと。
（注意事項等）
・個票データは、調査対象者個人の特定につながる可能性のある情報を含むため、一部を統計的に処理・加工し、個々の回答者が識別不可能な形式で提供します。
・データ提供までに１週間程度お時間をいただきます。
・個票データは、ファイルサイズが20MBを超えるため、PrimeDrive（ファイル転送システム）を使用して送付させていただきます。DLリンクは期限が最大で3日間となりますのでお早めのダウンロードをお願いいたします。
・２週間以上（年末年始、土日祝除く）経過しても連絡がない場合は、申請いただいたメールアドレスが誤っている可能性等がありますので、改めて申請いただくか、内閣府政策統括官（経済社会システム担当）付参事官（総括担当）までお問い合わせください。
・申請内容によっては提供できない場合があります。

Access to the micro-level data of the “2019 to 2021（First to Third） Survey on Satisfaction and Quality of Life” will be granted upon your agreement to the terms of use set out below. If you wish to obtain the data, please review the terms of use and complete the required fields in the application form provided below. Once your application has been verified, the micro-level dataset will be sent to the email address you provided.
Terms of Use
１．You shall not attempt to identify individual survey respondents using the provided micro-level data.
２．You shall implement reasonable security and management measures to ensure the safe handling of the provided data.
３．The provided micro-level data shall be used solely by the applicant and their affiliated organization, and shall not be redistributed to any third party.
Notes
・The survey was conducted in Japanese, and the questionnaire is available only in Japanese.
・The micro-level data have undergone statistical processing to prevent the identification of individual respondents, as the dataset may contain information that could otherwise enable such identification.
・Please allow approximately one week for the provision of the data.
・The data will be provided via PrimeDrive (file transfer system), as the file size exceeds 20 MB. The download link will remain active for up to three days, and we kindly request that the file be downloaded within this period.
・If no communication is received within two weeks (excluding year-end/New Year holidays and weekends/public holidays), the email address entered in the application may have been incorrect. In such cases, please submit a new application or contact the Office of the Director for Economic, Fiscal and Social Structure, Cabinet Office.
・Please be advised that, depending on the content of the application, the data may not be provided in certain cases.
